Our goal is to provide you with the information you need so that your DOT Medical Exam can be completed as quickly and easily as possible. Appointments are available quickly, often same day. We are located in Venice, Florida with lots of parking space for your big rig if needed.
What To Bring With You:
- Bring a list of your medications, bringing the bottles is preferable.
- Review the Medical History described in detail below, if there are any "yes" questions then on the right side of the page you will see the information you need to bring with you to make this process quick and easy.
If you are a smoker:
- If you are a smoker and over the age of 35, please let us know when you make the appointment. A Pulmonary Function test is required, extra time is required and there is an additional charge.
Automatic Disqualifiers:
Some common issues that automatically disqualify a person from being able to get a CDL license.
- Alcoholism
- Having a Defibrillator
- Diabetes with insulin use, the driver may be certified if the diabetes is well controlled and the HgA1C is less than 10.
- Meniere's Disease
- Seizures
- Schizophrenia
The Health History Form
We are here to make this process as easy and painless as possible. When you arrive for your appointment, prior to the exam, you will need to fill out the Health History Form. On the example form accessible by clicking on the following link, there are questions on the left that you will need to answer. If any of the answers are yes, then you may be required to provide additional documentation. Please review the directions and bring what is required to ensure you can get ‘er done quick and easy and walk out with your certification!
